Permission Form

Level Breakdown for new and returning students:

PLAYer Level - 1) All New students 9 and younger; or 2)One year experience and younger than 9.(Must be 7 to enter program)

Par Level - 1) All New students 10 and older; or 2) Two years in First Tee program. 

Birdie Level - 1) Three years in program and Par certified.
